Under Illinois State Law (410 ILCS 535 – Vital Records Act) only specific individuals have legal access to birth or death certificates.  The City Clerk’s Office will issue certificates to authorized individuals ONLY.  Vital Records are not considered public information, nor are they subject to the Freedom of Information Act.

 

TO OBTAIN A CERTIFIED BIRTH CERTIFICATE, YOU MUST BE ONE OF THE FOLLOWING:

 

  • The person named on the record if 18 years or older
  • The parent, if their name appears on the Birth Certificate as the mother or father
  • An Agent having a Notarized Letter authorizing access
  • A court order
  • Other Legal Representative, i.e., Attorney acting on behalf of a person, a Power of Attorney, an Agent approved by the State Registrar as a Legal Representative of the child.  Written evidence of guardianship or legal representation is required.

 

Uncertified copies of birth records for genealogical purposes are available to individuals who may not otherwise be entitled to receive a certified copy, if the person’s date of birth precedes the current date of 75 years or more.  If a record has been on file for 75 years, anyone is able to receive a copy stamped “For Genealogical Purposes Only” non-certified.   Fee for non-certified genealogical copies is $6.00 each.

 

TO OBTAIN A CERTIFIED DEATH CERTIFICATE, YOU MUST BE ONE OF THE FOLLOWING:

 

  • The informant listed on the death certificate
  • The next of kin
  • Someone who has a personal or property right interest in the certificate
  • Executor or Administrator of the decedent’s estate

 

Death records are not public records.  If you are not a relative of the deceased person, a letter or document from the office or agency that needs the death certificate must accompany the request.  An uncertified copy of a death record is available to genealogical researchers if the death occurred at least 20 years prior to the date of the request.  Fee for non-certified genealogical copies is $6.00 each.

 

 

REQUESTS IN PERSON OR BY MAIL

 

To request birth or death certificates, please complete the required application below.  The application must be signed by the applicant and submitted with an acceptable form of identification (see below).

Requests can be made in person at City of Lincoln, City Hall, 700 Broadway Street – Monday through Friday, 9:00 a.m. to 5:00 p.m. (except for City holidays).  Payment can be made by cash or check payable to:  City of Lincoln.

 

Requests by mail must include a copy of an acceptable form of ID listed below along with the application to:  City of Lincoln, Attn:  Vital Records, P.O. Box 509, Lincoln, IL  62656. Payment must be submitted with the request by check, money order or cashiers check payable to:  City of Lincoln.

 

 

Acceptable forms of identification are any of the following:

Valid Driver’s License

Valid State Issued ID

Valid US Passport

Valid Military ID
